Evaluating automated electronic case report form data entry from electronic health records

Background:
Many clinical trials leverage real-world data. Typically, these data are manually abstracted from electronic health records (EHRs) and entered into electronic case report forms (CRFs), a time and labor-intensive process that is also error-prone and may miss information. Automated transfer of data from EHRs to eCRFs has the potential to reduce data abstraction and entry burden as well as improve data quality and safety.
Methods:We conducted a test of automated EHR-to-CRF data transfer for 40 participants in a clinical trial of hospitalized COVID-19 patients. We determined which coordinator-entered data could be automated from the EHR (coverage), and the frequency with which the values from the automated EHR feed and values entered by study personnel for the actual study matched exactly (concordance).
Results:The automated EHR feed populated 10,081/11,952 (84%) coordinator-completed values. For fields where both the automation and study personnel provided data, the values matched exactly 89% of the time. Highest concordance was for daily lab results (94%), which also required the most personnel resources (30 minutes per participant). In a detailed analysis of 196 instances where personnel and automation entered values differed, both a study coordinator and a data analyst agreed that 152 (78%) instances were a result of data entry error.
Conclusions:An automated EHR feed has the potential to significantly decrease study personnel effort while improving the accuracy of CRF data.

For this study, we adapted the Montgomery Borgatta Caregiver Burden Scale, used widely in the United States, to the Saudi Arabian context. To produce an Arabic, culturally sensitive version of the scale, we conducted semi-structured interviews with 20 Saudi family caregivers. The Arabic version of the scale was tested, and participants were asked to comment on the appropriateness of items for the construct of “caregiver burden” using the repertory grid technique and laddering procedure – two constructivist methods derived from personal construct theory. From interview findings, we examined the content of the items and the caregiver burden construct itself. Our findings suggest that the use of constructivist methods to refine constructs and quantitative instruments is highly informative. This strategy is feasible even when little is known about the investigated constructs in the target culture and further elucidates our understanding of cross-cultural variations or invariance of different versions of the scale.

Collaborations between community colleges, non-research centered universities and research universities can enrich the flow of students into Science, Technology, Engineering and Mathematics (STEM) majors and careers. The nation is beginning to understand the importance of such interaction especially with under-represented minorities and those with disabilities. For over fifteen years our group has developed new ways to integrate these students and their faculty to the research culture. This will lead to increased diversity and inform research university faculty of the great talent that is latent in these underserved pools.

Summary
Translations that file two compared entities under a third term are like commodities or exchange-value transactions. Here are several examples: “Pani” in Hindi and “water” in English are both local variations of H20 (Chakrabarty 2000: 75); the Australian Aboriginal churinga and a Santería toque are affirmations of community; nineteenth-century slaves' work-slowdowns in Alabama and the contemporary manufacture of paket-kongo in Haitian Vodou can be compared as subaltern resistance; the fact that a Dinka boy in Africa “has a ghost in his body” (Lienhardt 1957: 58) and that a Thai villager afflicted by a phii paub cries out or laughs loudly and then hides her face (Tambiah 1970: 321) are two members of the class spirit possession. Such translations into artificially constructed third terms are like commodities that repossess the object of study with different properties than those with which they began. It is intriguing to think of theoretical key terms as carrying traces not only of the geographies of their original disembedding, but also of the economic regimes they replicate in the forms of exchange they facilitate, as “factors” (in the word's original sense of agents of exchange), or currencies. As we will see, this is especially apropos for the case of “possession”, which quite directly links ideas about the body to ideas about property.
By contrast, translations that investigate compared phenomena through their direct contiguity and local terms in order to seek their shared properties in lived experience are akin to barter, or use-value transactions.

Summary
In the four months since September 11, “Ground Zero” as a site and “9/11” as an event have both become key symbols, acquiring layers of signification gathered around them. Many of these are central issues, or should be, for scholars of religion: the consolidation of complex, bewildering events into orderly and satisfying narratives in media representation; death and memory, or the problem of absence and presence; nationalism and religion, and their convergence in civil religion; and the perennial question of just what is “religious,” here in the form of the assertion that “this has nothing to do with ‘real’ Islam.” In this impressionistic essay I touch on each of these. The field reports derive from two two-week visits to New York City, the first in October 2001, and the second in January 2002.
At the end of October, those riding the southbound subway to lower Manhattan to shoot grim-faced photos of themselves in front of Ground Zero were mostly out-of-towners. The locals were riding north, to the Bronx and Yankee stadium, where a miracle seemed to be unfolding, divine intervention in the sacred economy that would bestow sports victory as compensation for the losses of the month before. Riding south to remember, north to forget. Not all spirits have retreated to metaphor, even in the very public sphere of U.S. electoral politics. As we learned during the last presidential campaign, Governor Sarah Palin enlisted the help of Kenyan pastor Thomas Muthee during his 2005 visit to Alaska to cast out the spirits that hindered her career. The New York Times elaborated, “Ms. Palin has long associations with religious leaders who practice a … brand of Pentecostalism known as ‘spiritual warfare.’ Its adherents believe that demonic forces can colonize specific geographic areas and individuals.… Critics say the goal of the spiritual warfare movement is to create a theocracy” (Goodstein 2008, my emphasis).

The essay evaluates the general problem that, while most modern republican constitutions follow the U.S. and French models in declaring religious freedom, absolute religious freedom is impossible and undesirable. How are religious freedoms constrained, and how much should they be? The essay evaluates the strategies by which limitations on freedoms of religion are constructed and imposed, especially the powerful isomorphism of law and science described by Boaventura de Sousa Santos. Taking the example of Afro-Brazilian religions in relation to the Brazilian state since 1890, post-emancipation, the essay argues that pseudo-scientific discourses of “public health” constrained the religious practice of former slaves, thus allowing the trompel'oeil of religious freedom to continue in the new republic, even as freedoms were in fact constrained by the state.
